The role of co-creation for sustainable large-scale adoption of digitally supported integrated care: prehabilitation as a use case.

We are very pleased to inform you that the manuscript “Role of co-creation for large-scale sustainable adoption of digitally supported integrated care: prehabilitation as a use case”, in which Ramon Martinez de Mariquita, as a member of Stimulo, has participated, has been accepted for publication in the International Foundation for Integrated Care (IFIC) journal.

Gradually, the industrial design agency “Stimulo Design” is turning its design expertise towards evidence-based co-creation combining appropriate methodologies and a structured evaluation framework. The effectiveness-effectiveness gap is a well-known constraint to the adoption of digitally enabled integrated care services. This report describes the co-creation process carried out (2016-2021) to deploy a prehabilitation service at the Hospital Clinic of Barcelona with the ultimate goal of achieving sustainable adoption and facilitating the transferability of the centre.

The current report provides three very definite outcomes. First, it illustrates the potential of evidence-based co-creation, specifically using Design Thinking methods, and quality improvement methodologies with iterative PDSA cycles to achieve large-scale implementation of integrated care services for chronically ill patients, taking prehabilitation as a use case. As a second outcome, it identified factors influencing prehabilitation outcomes and determinants of service adoption, using the CFIR framework. Finally, based on the lessons learned, we propose a list of Key Performance Indicators for long-term quality assurance of the intervention after adoption. Overall, the co-creation approach shows high potential for service refinement in other complex health care interventions.

After 4 years of research, the Hospital Clinic of Barcelona has launched a prehabilitation programme to reduce post-surgical complications in high-risk patients. The challenge is to involve users in improving this service and to extrapolate this experience to other hospital sectors.
